Please sign our online petition to the New York City Mayor, Department
and Mental Hygiene, and finally, the City Council. The petition has
been created so that we can demand that “informed patient consent”
(or at the very least, informed patient notice) be incorporated as part
of the NYC A1C tracking plan by the NYC Dept. of Health and Mental Hygiene.

You can also direct any questions, comments, concerns or frustrations
you may have about the mandatory A1C tracking plan to the New York City
Department of Health and Mental Hygiene officials managing this initiative.
Do not hesitate to write, fax, e-mail and/or call them about this plan
and do not permit them to intimidate you! Remember, these officials
work for taxpaying New York City residents as public employees.
